I've never had anyone decline, but I've also never done it in public (like a shop or mall) since I don't go out much atm. One guy who didnt have a mask pulled his jacket collar up over his nose and mouth while he was near me. I am a generally patient person and can put up with a lot, but I have NO patience for people who are risking my life and the life of those around them by not wearing a mask in a close work environment during a pandemic. I hold up my hand if they're approaching me, and I say "Excuse me, could you please pull your mask up?" Usually (at work) they do so, and then we just continue on with what we were doing.

In a friendly voice: Excuse me, would you please put your mask on so it covers your nose too? Maybe you feel it's annoying and unnecessary for yourself, but I have a vulnerable health, so I can't take risks. And I don't have a car, so I have no other option to take this train today.
